According to Jackson, there are currently six of them in the game, including the Melted Heart of Selig amulet, the Ring of Starless Skies, the Harlequin Crest and Andariel’s Visage helmets, The Grandfather two-handed sword, and the ominously-named Doombringer sword.
The above items have a chance to drop from enemies above level 85, and the item power is 820. Apparently, they can be obtained from anywhere that regular unique items can be obtained. Jackson explained that any unique item drop will likely be replaced by one of these six rarities, though he noted that "they're really, really rare!"
He advises players to focus on content that offers the highest number of unique drops within a certain time frame. He didn't say exactly what to focus on, however, so you'll still have to do some legwork.
Given the rarity of these items, it's likely that only the most dedicated and hardcore players will put in the time and effort to acquire them, unless of course you're lucky.
